  • How do I order an ID card or order a replacement ID card?
    You can order a replacement ID card by registering and logging in to your MyPortal Itransact website or by calling customer service specialist.
  • Who do I contact if I have a claim issue?
    You may call us toll-free 1-877-493-6282 and speak with a live representative.
  • When will I receive my Dental cards and what do they look like?
    Max Care Dental provides fulfillment upon your effective date. They will arrive within 10-14 business days of your effective date. This includes a perforated identification card on the back page for your member’s use. Booklets/Cards can be sent to the employee’s home address with receipt of the proper information or to the employer for delivery.
  • Who do I contact to verify my Dental coverage?
    Members may call customer service Toll-Free 1-877-493-6282 and use the (Member Services Prompt).
  • How do I add a spouse or dependents to my plan?
    In order to add a dependent to your plan, it must be during open enrollment or the insurance company requires proof of a Qualifying Life Event. These are any major life changing events that provide documentation, such as the loss of existing coverage, loss of coverage with employer, becoming a U.S. Citizen, Marriage Certificates, Birth Certificates, and more.   • What if my provider does not recognize Max Care Dental?
    Your provider will most likely recognize the name of the network rather than the name of the plan. Max Care Dental is a PPO plan that utilizes the Plus PPO network. Plus Network is a combination of three dental networks; Dentemax, United Concordia, and Careington. The combination of these networks has created one of the largest dental PPO network, with over 600,000 providers in the United States.
  • How can I cancel my policy?
    We value each of our members and strive to provide the best service available. We understand that circumstances can change. If you wish to end your services, please contact our Member Care department. We will ensure that your account is properly closed, and provide you with a cancellation confirmation for your records.
